Position Summary: Our Client is seeking an RN Nurse skilled in Case Management. An RN Case Manager (Registered Nurse Case Manager) is a healthcare professional responsible for coordinating patient care, and ensuring that individuals receive appropriate services across different settings. They assess patient needs, create personalized care plans, manage resources, monitor progress, and collaborate with healthcare teams to improve outcomes. RN Case Managers often work in hospitals, insurance companies, or home health agencies, ensuring efficient, quality care while controlling costs and optimizing patient healt h. Key Responsibilities: Assessment : Evaluate patients’ medical, psychological, and social needs. Care Planning : Develop and implement personalized care plans.
